Community Service Block Grant Act (CSBG) requires Community Action Agencies to develop three year Community Action Plans which include strategies for measuring program results. These Plans must include:
- Agency’s Mission Statement
- Community Profile
- Community Needs Assessment
- Description of Agency’s Service Delivery System
- Identification and Evaluation of National Indicators and CSBG National Goals and Outcome Measures
- Description of Agency’s Linkages to other local organizations/service providers
- Description of Agency’s Funding Strategy
- Agency’s Vision Statement for the next 3 years
